Submitting a Claim

Please submit any tort claims you wish to assert against Lakehaven Water and Sewer District to our Claims Agent, Lisa Alexander, during the hours of 8 a.m. to 4:30 p.m., Monday through Friday.

  1. Review the Instructions for Filing a Claim for Damages (PDF)
  2. Download and print the PDF version of the Claim for Damages Form (PDF) 
  3. File the completed claim to the address below. 
     
    You may submit the form by mail. (We are accepting claims in person during the pandemic).
